Nathan Landsbaum President & CEO of Home State Health (Centene’s Missouri Plan) and Dr. Marcus Zachary, Global Lead of Value Based Care Babylon Health describe their risk-based partnership to deliver access to primary and comprehensive care to patients in 10 counties in Southeast Missouri. Babylon serves as the Primary Care Physician via its mobile application-based telehealth platform. Home State Health members assigned to Babylon have access to their PCP through their smartphone any time, day or night.
Next, Dr. Jerry Dwyer, a cardiologist and Coala’s Clinical Board Advisor, introduces Coala Life and how it is transforming cardiac care by enabling vulnerable patients and their connected providers monitor and manage their cardiac health at any time from anywhere. The remote, real-time cardiac monitoring is being used pre and post-procedure and shows a decrease in symptoms of palpitations, anxiety, depression, and improved quality of life. Dr. Dwyer is joined by Liane Parker, RN, CPHM, who will provide perspective on how this patient-empowered innovation for underserved communities enables real time and longitudinal management that improves outcomes for hard to reach patients and alleviates worries for patients and their care-givers. Joining the discussion is a patient using the Coala device and sharing their experience with the device and program.
